Yes, you can send me an occasional email about shop updates

I certify that I am aged 18 years or older

I fully understand that variations may occur between the image provided or chosen and the actual tattoo when applied to my body and that tattoos will fade over time and with exposure to sunlight

I am not prone to fainting or vertigo

I am not allergic to latex, soap, metals, or tattoo pigments

I am not pregnant or nursing

I have not consumed any anticoagulants within the last 24 hours (such as aspirin or ibuprofen)

I do not have medical conditions that may affect the services, including hepatitis, diabetes, heart disease, epilepsy, hemophilia, high blood pressure, or other physical conditions that may affect application or healing.

I am not under the influence of alcohol or drugs

I do not have any plans to soak in water including baths, pools, lakes, the ocean, etc. for 2 weeks after tattoo application

I have been or will be given instructions on the care of my tattoo while it's healing; and I understand them and will follow them explicitly.

I understand that a tattoo is a permanent change to my appearance and can only be removed by laser or surgical means, which can be disfiguring and/or costly and which in all likelihood will not result in the restoration of my skin to its exact appearance before being tattooed.

I have been fully informed of the inherent risks associated with getting a tattoo. I fully understand that these risks, known and unknown, can lead to injury, including but not limited to, infection scarring, difficulties in detecting melanoma, allergic reactions to tattoo pigment, latex gloves, and/or soap. Having been informed of the potential risks associated with getting a tattoo, I still wish to proceed with the tattoo application.

I will not pursue legal action about the services provided
